Preparing for your appointment

Telehealth Appointment:

• You will receive an email to let you know that your child is now eligible to book in their one year telehealth appointment, which can be booked either via a phone call, or the ORIGINS app.
• We will give you a follow up call if you are yet to book your appointment.
• Telehealth instructions will be shared once your appointment is booked in.
• This appointment has to be completed first before your allergy appointment can occur.

Allergy Appointment Checklist: 

Things to bring to your appointment:

• Please bring your child’s purple book with you.
• You may also want to bring in a snack and your child’s favourite toy.
• A biological sample pack will be posted to you before your appointment. Please collect these samples on the morning of your appointment (apart from stool- which can be collected at any time and frozen). If these have not been collected, please bring your sample collection packs to your appointment. If you have not received any sample collection packs please contact us at ORIGINSPaeds@telethonkids.org.au or call (08) 6375 6300. 

Key points to remember: 

• Avoid using antihistamines 5 days prior to your appointment. If antihistamines are required/have been given or your child is unwell, please contact the team to change your appointment. 
• Collect details about your partner’s height and weight (if applicable) and make a note of them in your phone so you have them with you on the day.
• Please also bring in any samples you have collected at home throughout the year (e.g any stool samples in your freezer).

• Bathers (please no rashies or long-sleeved bathers) for your child. This is for the “Spaceship” ride in our Bod Pod. Note, your child will not get wet, this is to reduce the impact on the measurement.
• Your child’s favourite toy and a snack & water bottle

Key points to remember: 

• Avoid using antihistamines 5 days prior to your appointment. If antihistamines are required/have been given or your child is unwell, please contact the team to change your appointment.
• Our team will provide you with biological samples packs for your child when you arrive at your appointment. These can be collected during your appointment. 
• Collect details about your partner’s height and weight (if applicable) and make a note of them in your phone so you have them with you on the day.
• Ensure you complete the following online assessments before your appointment, which you would have received via email, but are also available via our ORIGINS App;
  • Conner’s Early Childhood questionnaire
  • Ages & Stages questionnaire
    • If you are unable to locate these questionnaire emails- please contact us via email or phone
    • Every sample and questionnaire completed generates an entry into the monthly draw for a $100 Coles/Myers voucher
